Dr. David Ores and Carlos are giving former gang members and ex-cons a fresh start.

"Fresh Start" is a New York City-based community program that medically removes tattoos based on gang and prison affiliation for FREE. Individuals must qualify first and then "Fresh Start" will remove tattoos from the face, hands and neck area that might interfer with the job interview process.

Prison and gang tattoos, that are visible, inhibit many ex-cons and gang members from getting jobs and moving on with their lives. For this reason, Carlos, a former gang member has joined forces with Lower East Side Doctor David Ores to offer this free service to help people begin again. It also helps people who could not normally afford a tattoo removal procedure, which can cost hundreds of dollars. Dr. Ores and Carlos asked for Help Me Howard to put the word out about their program and we were only too happy to help.

Dr. Ores is a General Practitioner, who also does laser hair removal.

My story concerns the Kings Highway Center of Preferred Health Partners, HIP PRIME HMO, EmblemHealth Company and the down right plain stupidity of the employees at the Kings Highway Center.

First let me start off with my member ID # 14385319. On Tuesday July 13, 2010 at around 10:00 A.M. I called my Dr.'s office at the Kings Highway Center to refill two (2) prescriptions and two (2) prescriptions only. My Dr. is Dr. Michael C. Crismali. His phone number is 718-615-3777. When that number is called, the caller is connected to the switchboard. The caller must go through the switchboard and also must go through a security briefing before he or she is switched to the Dr.'s office. Upon getting through to the Dr.'s office I was able to speak to one of the Dr.'s assistant. I told her I wanted to renew two (2) prescriptions. She put me on hold. I was on hold for 10 minutes. I hung up after 10 minutes and decided that I would go into the Dr.'s office in person that afternoon with the two (2) prescriptions.

At around 2:00 P.M. that same afternoon I went into the Dr.'s office. I put the two (2) bottles of pills along with my ID card on the desk. The two (2) bottles of pills were:
1) Simcor 500/20MG. 1 tablet daily
2) Idomethacin 75MG. 1 tablet twice daily
The nurse/receptionist sitting at the front desk asked me what I wanted. I told her I need to refill these two (2) prescriptions. She wrote down on a piece of paper all the information from the two bottles. I even saw her write down the information on a piece of paper. The information was as follows:
1) Simcor 500/20MG. 1 tablet daily
2) Idomethacin 75MG. 1 tablet twice daily
She told me she would give it to the Dr. I said thank you and I walked out.

The next day which was Wednesday July 14, 2010 I called up Dr. Michael Crismali's office at around 11:00 A.M. to find out if the prescriptions were called in to the mail order company I use which is Innoviant. Once again after going through two (2) security briefings and finally getting through to the Dr.'s office I was once again put on hold for 10 minutes and no one ever got back to me. Since I was going to the Center that afternoon to see the eye Dr. I figured I would stop by Dr. Michael Crismali's office to see what was happening with my two (2) prescriptions. As I was getting out of the elevator I saw the nurse who I gave my two (2) prescriptions to the day before. She said to me, "the Dr. has your two (2) prescriptions". I told her, "Gee, it would be nice if you would let someone know". She laughed at me and said, "We don't have to go.

On Friday July 16, 2010 I called my mail order company Innoviant to see if they had gotten the two (2) prescriptions yet from the Dr. I was told that there were no prescriptions called in. Well, I was furious to say the least. I had dropped off the two (2) prescriptions on Tuesday, it was now Friday and the prescriptions were still not called in. How long does it take to pick up a phone and call in prescriptions. After all, Dr. Michael Crismali is no brain surgeon. He does not do surgery 24/7. I walked into his office at around 10:00A.M. mad as all hell. When I walked in I said, "When the hell is he going to call in my prescriptions or does he need help in dialing a phone. Not one person in the office answered me. Well now by blood pressure was about to burst. I said, "Does anyone understand english or are you all a bunch of idiot's"? I walked out of the office and went downstairs to member services. I spoke to a woman in member services who had no answer at all for me. A real idiot. She told me to go to the third floor and talk to Mr. Lewis the Administrator of the Kings Highway center. While I was in Member Services, Francine from Dr. Michael Crismali's office came in and dropped on the desk a sheet listing 8 prescriptions which were just faxed to Innoviant. My question was, "What the hell is this? I asked fot two prescriptions, not 8". I was told to go to the third floor and talk to the Administrator, Mr. Lewis.

While talking to Mr. Lewis I kept asking him why 8 prescriptions were faxed when all I asked to be refilled were two prescriptions. He refused to answer the question. One of the prescriptions listed was Fortamet 500MG. 1 tablet daily. I never heard of that prescription. I kept asking him what that one was. Again he refused to answer that. At this point in time he began racial profilling at me. Every other word out of his mouth was YOU PEOPLE. Not once did I use the "N" word towards him, but he kept referring to me as YOU PEOPLE. He used the word YOU PEOPLE at least 10 times. When I told him that's racial profilling he just went ahead and once again used the word YOU PEOPLE.

Now I will point out the down right stupid mistake that the nurse/receptionist in Dr. Michael Crismali's office made when she faxed over 8 prescriptions instead of the two that I needed to Innoviant on Friday July 16, 2010. I needed:
1) Simcor 500/20MG. 1 tablet daily
2) Indomethacin 75MG. 1 tablet twice a day
Instead she faxed to Innoviant the following:
Idomethacin 25MG. 1 tablet 3 times a day.

As for the prescription Fortamet, I later found out that it is the same as Metformin which I do take. I do not get Metformin from Innoviant. I get it from my other HIP plan. The reason why I get it from my HIP plan is because I do not pay for it. If I was to get it from Innoviant I would end up paying $20.00 for it every time I would renew it. I never told the idiot to renew Metformin (Fortamet). She took it upon herself to renew it. Now, get this. I take Metformin 500MG. 1 tablet twice daily. She faxed in Fortamet 500MG. 1 tablet daily. Where the hell did she get that information from? Now, for the next one:
I take Tramadol 50MG. 1 tablet every six hours when needed for pain. She faxed in Tramadol 50MG. 1 tablet three times a day.

I have a question, did she do this with the knowledge of Dr. Michael Crismali or did she do it on her own with whatever little amount of brains in her head? What kind of idiot is that to be playing games with someone's health. I had to call Innoviant on Friday July 16, 2010 and correct the entire order. I spoke to Elaan Radley a Pharmacist with PrescriptionSolutions. Her number is 1-888-226-6779ext. 51216.

As for the racial profilling of an Administrator at the Kings Highway Center, isn't it against the law to engage in racial profiling? I mean, if I was to go into the Kings Highway Center and start using the "N" word, what would happen to me?

On Friday July 16, 2010 I filed one (1) complaint with HIP Customer Service.
1) Complaint # 68464005.1 - dealing with the mistakes with the prescriptions, the amount of prescriptions that was faxed to Innoviant and the racial profiling by Mr. Lewis.

On Monday July 19, 2010 I spoke to Tina Davis a Supervisor with HIP Emblem Health Customer Service in Ohio. She told me that my complaint was sent to provider services and they will take care of it. She went on to tell me that they will not be in touch with me, nor will they tell me what action was taken if any against those two (2) employees. So, where does that leave me? How the hell can I go to that Dr. ever again. As far as I'm concerned no one in that Office can be trusted to do the right thing. And guess what, they can do whatever the hell they want and they will get away with it. If that does not suck, what does?
